BMI Reporting Opt-Out

As occurs every two years, the SVE school districts has been selected to take part in a BMI survey by the New York State Department of Health. It is required we report to them information our students’ weight status groups which is the height, weight and calculated BMI that is part of a student’s required school health examination. For this 2020-2021 school year it will be students currently in grades 1, 2, 4, 6, 8, 10 and 12th grades from data on their previous year’s health exam or physical. No names or additional information about any individual student is sent. You may choose to have your child’s information excluded from this survey report. To opt-out, print and complete this form, and return it to the school nurse by 1/2/2020.
Birthday Celebration Order Form

The Food Service Department is offering a new service this year to help you easily celebrate your child’s birthday at school. The school cafeteria will now offer healthy, peanut-free, fun treats you can easily purchase for the entire class.

S-VE announces its participation in the Summer Food Service Program (SFSP). Meals will be provided to all children 18 years and under without charge. In accordance with Federal civil rights law and U.S.Department of Agriculture civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.
