BMI Reporting Opt-Out

As occurs every two years, the SVE school districts has been selected to take part in a BMI survey by the New York State Department of Health. It is required we report to them information our students’ weight status groups which is the height, weight and calculated BMI that is part of a student’s required school health examination. For this 2020-2021 school year it will be students currently in grades 1, 2, 4, 6, 8, 10 and 12th grades from data on their previous year’s health exam or physical. No names or additional information about any individual student is sent. You may choose to have your child’s information excluded from this survey report. To opt-out, print and complete this form, and return it to the school nurse by 1/2/2020.

S-VE announces its participation in the Summer Food Service Program (SFSP). Meals will be provided to all children 18 years and under without charge.
