All enrolled students in our school district via CEP, regardless of income level, are eligible to receive a healthy school breakfast and school lunch at no charge each day for the 2024-2025 school year. Please see below for more information.

Does your child have food allergies or require some form of meal modification? Please select the appropriate choice below and complete the necessary documentation.

       Complete One Form:
Parents or guardians must choose one of the following forms:

Request for Meal Modification Form (includes a doctor's note).

or

Meal Modification Medical Statement (completed by a healthcare professional).

Submission:
Once the chosen form is completed, it must be returned to the school nurse at the child’s school.

Important Note:
Completing both forms is unnecessary—only one is required to initiate the meal modification process.
